Scrub typhus is a leading emerging etiology of febrile illnesses in children in India. Varieties of clinical manifestations of scrub typhus ranging from mild, self‐limiting state to severe disease lead to morbidity and mortality. To evaluate the outcome and clinical presentation of scrub typhus infection in acute encephalitis syndrome children in our area. This was a cross sectional observational study, conducted in the Department of Paediatrics in Gandhi Memorial Hospital, Rewa, M.P, India. All children up to 15 years of age with acute febrile illness were enrolled in this study. All socio‐demographic data and clinical presentation were analysed, diagnosis was confirmed for scrub typhus using Enzyme‐Linked Immuno‐sorbent assay (ELISA) and detect serum IgM antibodies against Orientia tsutsugamushi antigen. Results: out of total 110 patients 56 (50.9%) cases were tested positive for scrub typhus. Most of the Scrub typhus patients (62.5%) were male and male female ratio was 1.67. Majority of the children was 1‐5 years of age, mean age was 7.83±4.12 years. Predominant cases occur in the mansoon season. Fever and Altered consciousness were the most common clinical presentation seen in the 100% cases. Among laboratory features of scrub typhus patients, thrombocytopenia followed by raised liver enzymes (SGOT, SGPT) and raised serum bilirubin was common. Conclusion: Scrub typhus is the common causes of acute febrile illness in children, may cause high morbidity and mortality if not treated early.
