A Comparative Study of Endoscopic Versus Conventional Septoplasty in DNS

Abstract

To compare the outcomes of both endoscopic septoplasty and conventional septoplasty techniques in terms of duration of surgery, complications and symptom relief by using NOSE scale Methods: Our study included 84 subjects with 42 in each group. Detailed clinical examination and DNE was done prior to the surgery. Symptoms severity were assessed by NOSE score. Patients were followed up for 6 months. At the end of 6 months post operative DNE was done and NOSE score was used to assess the relief of symptoms. In this way we compared the outcomes subjective and objective perspective. Results: The most common age group undergoing septoplasty was in 3rd decade in our study. Males were affected more than females with 1.8, 1 male to female ratio. Nasal obstruction was the most common presenting complaint followed by headache, nasal bleeding, nasal discharge and hyposmia was the least common symptom. Posterior septal deviation is the most common type of deviation followed by anterior septal deviation and high septal deviation. Septal spur was the least common type. Endoscopic septoplasty is comparatively a longer procedure when compared to conventional septoplasty but it was not statistically significant. Posterior septal deviation and high septal deviation was better corrected with endoscopic septoplasty in our study. Caudal dislocations were better corrected with conventional septoplasty There were only fewer complications like septal perforation and synechiae in endoscopic septoplasty which was not statistically significant. In both conventional septoplasty and endoscopic septoplasty there is improvement in symptoms as measured by using NOSE score. But the difference between the groups was not statistically significant. Endoscopic septoplasty was a longer procedure compared to conventional septoplasty. Posterior and high septal deviations were better treated with endoscopic septoplasty. Since it is done with digital monitor system it is an effective tool in teaching.
