Clinical Study of Fetomaternal Outcome in Pregnancy with Anemia at a Tertiary Hospital

Abstract

Anemia is associated with 22% maternal death around the world. India contributes to about 80% of maternal deaths due to anemia in south Asia. Present study was undertaken to compare the maternal and neonatal outcome in two groups based on hemoglobin concentration and severity of anemia. Present study was single‐center, prospective, observational study, conducted pregnant women, with single intrauterine pregnancy, cephalic presentation. 300 patients were further divided into two groups as cases (Hb < 11 gm %) and controls (Hb > 11 gm %). Prevalence of anemia among pregnant women in our institute was 37%. In our study majority cases (41 %) and controls (57 %)were from 21‐ 24 yrs age group. 93 cases with mild, 48 cases with moderate and 9 cases with severe anemia. Patients with severe anemia had more proportion of women with higher parity. It has been found that there was increase in severity of anemia with increase in parity. We had 33% patients with mild anemia, 42% with moderate anemia and 44% patients with severe anemia had birth interval < 2 years. There were 105 cases and 81 controls had vaginal deliveries, 44 cases and 67 controls underwent c‐ section and there were 1 instrumental delivery in cases and 2 in controls. 25% of patients with mild anemia had preterm delivery, compared to 33% with severe anemia. 57 neonates of cases and 37 neonates of controls had birth weight less than 2.5 kg. this difference is statistically significant, showing that low birth weight was more common in anemic patients. Perinatal mortality noted in 3 cases. Severe anemia was associated with significantly more maternal and perinatal complications such as postpartum hemorrhage, more requirement of blood transfusion and parental iron, low birth weight, IUGR and prematurity.
